This series consists of 12 reels of 16 millimeter microfilm containing copies of completed Life Membership and In Memoriam enrollment forms from 1919-1946 for the Women's Missionary Society (WMS) of the United Lutheran Church in America (ULCA). They were generated in the course of recording those persons honored with a life membership in the WMS, or for whom memorials were to be given to the WMS. Labeling on the film refers to the records as indexes. They are arranged chronologically by year, with approximately 1- 3 years per reel. Within each year it appears the records are first arranged quarterly and then by synodical society. In some instances these quarterly groupings are arranged alphabetically by synod, but this is not always the case. The film contains a copy of each enrollment form completed, per quarter, by a synodical society and sent to the WMS general treasurer. Reel 4 contains an index from 1919-1929 for the New York synod, as well as the regular index for 1940. Within each year's quarterly groupings, the forms do not appear to be in strict chronological order. There are separate enrollment forms for life members and memorials. Life membership forms contain space for information regarding the honoree's name, address, who he or she is being presented by, and the address of where to send the certificate. There is also a space labeled "society and church," but it is not clear if this is the society and church of the honoree or the society and church honoring the person. Memorial forms contain space for information regarding the name of person being memorialized, his or her last address, who he or she is being presented by, and an address of where to send a certificate. The "in memoriam" form also contains the "society and church" field, but it is unclear on this form also whether or not it is the society and church presenting the memorial, or the society and church of the honoree.
